Clubfoot, Double-Chin, Hemophilia, Dwarfism, Hunchback and Cleft Palate should all be selectable on character creation. See, from how I see on /tg/ dwarfism actually let's you move faster, and climb tables with no need for click n drag. Only, way I could see it as a debuff if you gave the person a dwarf beard...
